DESCRIPTION:Share what the health equity efforts in your Health District/county have looked like during the VT CHEP grant. Highlight your problem statement\, examples of  how your teams have aligned your team policies\, practices\, mindsets\, resource allocations\, decision-making processes\, and capacities with the goal of ensuring everyone has a fair and just opportunity for health and wellbeing\, and if possible\, provide space for funded community projects to share what they have been working on. The VT CHEP suggests videos that are five to ten minutes in length. Any recording platform can be used. If you need assistance with recording and video sharing\, please reach out to Kyra and Cathy. \nThese recordings will be posted to the VtPHI’s Youtube channel and linked on the VT CHEP “Backbone Partners” webpage for the public to view. \nAll materials due to Kyra by Friday\, September 29th. They will be posted by Friday\, October 6th.
