VT CHEP: Health Equity Self-Assessment Open Discussion

Audience: Backbones, Integrators, District Directors
Purpose: To help reduce the deliverables for the Integrators and Backbone teams at the end of the VT CHEP grant, we are asking districts to complete the final Health Equity Self-Assessment (see attached Word Doc) by Monday, October 2, 2023. This will not be required again as part of the final assessment materials due on December 6, 2023.
The Health Equity Assessments are not tied to your funded community projects. It is designed to be a useful tool for your collaborative teams/coalitions/AHCs/steering committees to use to guide discussions on how you are working together on health equity. While this is only required a second time, by October 2, the VT CHEP’s hope is that this tool will be used as a check-point throughout your work.
